Founded in 1987, People of Color Against AIDS Network, also known as POCAAN, is a multicultural AIDS prevention organization that focuses on minority communities. It administers Konnect II, which provides referrals, information and education for newly diagnosed HIV-positive individuals. The organization also provides infant mortality prevention programs that work to decrease the incidence of infant deaths in African-American, Native American and low-income communities. The organization s Breaking the Chains: Focus on Freedom program partners with treatment and health providers, recovery and harm reduction leaders, prevention specialists, community members and addicts to develop programming for the well-being of substance users. People of Color Against AIDS Network maintains locations in Seattle, Tacoma and Yakima, Wash.
