What is Home Health? The goals of Home Health Care are to promote and maintain health, provide comprehensive care of the sick or disabled in their own home and prevent re-hospitalization and thus reduce the cost of health care. Care is provided in the home through intermittent skilled care visits. Our team supports families so that they can fully participate in the care of the patient within the home. We also help patients and families make use of appropriate community resources for prevention, maintenance and treatment of their various needs. What is Hospice? Hospice is a special "concept" of care provided to patients and families facing a terminal illness. Hospice programs are recognized for their ability to ease the death and grieving experience, and for providing an alternative to traditional care in a hospital setting. At Memorial Healthcare, our goal is to maintain the patient's quality of life by focusing on supportive care for the terminally ill patient and their family. The patient and family are both included in the care plan with special attention given to the emotional, spiritual and practical support needed and upon the expressed wishes of the patient and family.
