Whenever necessary to extend beyond these parameters with additional diagnostics (labs. XRAYS. ETC). We CAN usually report the added cost to YOU prior to the work being done. OUR ultimate goal to provide accurate COST-OF-SERVICES before YOU actually incur the expense with NO billing surprises AT a later date. Yearly EXAM
YOUR annual membership FEE provides YOU with a 1 hour comprehensive evaluation. This includes general screening. Chronic disease assessment. Preventive medicine. And lifestyle management AS indicated. Data includes labs. EKG. And FUNCTIONAL/FITNESS assessment. The member gains direct access to the provider effectively 24/7/365 (within reasonable defined parameters). Enrollment of ONE adult PER Family gains the children (above AGE 2) of the household access to OUR urgent care services Also. Episodic VISITS
THERE are multiple scenarios for acute illness ("sick call") management: -for remote advice (phone. Text. Email). There is generally NO charge, a cost of $30 is generated if a prescription is necessary. -office visits for common problems (EG. Cough. UTI. Abscesses) are $50. This appt is guaranteed SAME-DAY/NEXT-DAY and reserves YOU AT least 30 minutes. Almost ALL labs necessary for acute illness are performed on-site and are included. AS well AS injections and procedures. -minor ortho injuries requiring a splint are billed AT _____. X-RAYS are OUTSOURCED OFFSITE and are charged separately. -complex problems (chest pain. Dehydration. Acute respiratory difficulty. ETC) are billed AT $100. This includes a lengthy visit. More costly data (much of which we CAN DO onsite) and advanced treatments such AS IV therapy. Many chronic diseases benefit from periodic reassessments. Usually biannually or quarterly (EG. Diabetes. Lipids. HYPERTENSION). THESE visits are $40 and include PROBLEM-SPECIFIC labs if NECESSARY. PROBLEMS are only billed once. The initial encounter cost includes ANY necessary FOLLOW-UPS for the same or associated problems. The physician will personally discuss test results with YOU when they become AVAILABLE. CLEARANCE PHYSICALS
SPORTS. School. And work physicals are performed for members and their families AS needed for free. This does NOT include DOT EXAMS. VACCINES
IMMUNIZATIONS are custom charged. We have special FLU shot offerings for OUR MEMBERS. LABS
URGENT care labs are typically performed onsite and included in the visit cost. More complex labs are OUTSOURCED and require 2-3 days. When possible. This is factored into the cost of the visit. Occasionally. Additional charges MAY BE incurred. We will advise YOU of the charge difference prior to ordering the test. RADIOLOGY
RADIOLOGIC studies (X-RAYS. CT scans. Ultra sound. MRI. MAMMOGRAMS. ETC) are OUTSOURCED locally. The performing facility will bill YOU (or your insurance) directly and separately from OUR clinic COST. OFF-SITE CARE
CARE performed by other providers including pharmacies. Diagnostic centers. Specialist referrals. Physical therapy. Hospitals. And emergency departments will bill YOU (or your insurance) directly and independently from OUR clinic. In absentitia
we MAY BE able to help YOU even when YOU travel. Often problems CAN BE handled remotely and we are glad to DO it. When its minor or when in doubt. Contact US first. Conversely. Remote access with your professional typically remains intact even if we travel. If a face to face assessment is necessary. Arrangements will BE made with a covering provider on your behalf.
