LIFE St. Francis is the newest healthcare choice for seniors living in Mercer County and portions of Burlington County. We are located at 1435 Liberty Street in Hamilton and are focused on enabling seniors to live safely at home while being helped by a team of healthcare experts. Our care is comprehensive, individualized and provides a safe alternative to nursing home care. Because LIFE St. Francis provides and is responsible for all of your care, you may be held financially responsible for any care you receive outside the program that is not approved by the LIFE St. Francis interdisciplinary team. Our services cover all primary and specialty medical care, physical therapy, occupational therapy, durable medical equipment, prescriptions and transportation to our center and to all medical appointments. While visiting our center several days a week, participants receive breakfast, a hot lunch and a snack in addition to a host of fun activities and socialization.
