Golden Years has provided skilled nursing and rehabilitation in the community of Falcon since 1965 and provides a complete continuum of care including short term rehabilitation, long term care and out-patient therapy. Our professional and compassionate staff offers care and activities in a home like atmosphere where residents feel comfortable and secure. Following admission, a patient undergoes assessments within twenty-four hours from an interdisciplinary team including nursing, physical, occupational and speech therapy. Based on these assessments, a plan including our state of the art rehabilitation services is developed in conjunction with the patient and their family.
