(Walmart, Walgreens, RiteAid, Target, and more. Savings on major dental services such as: Dentures,Orthodontics, and Cosmetic Dentistry are included. Cosmetic discounts vary. MD Plus: $25 a month for the entire household, related or not. Includes: TeleMedicine, Hospital Advocacy, Ancillary services and prescription program. Combo Pack: $40 a month for the entire household, related or not. Both plans purchased as a combo pack. *Registration fees: Dental and Md plus fee is $20.00. Combo Pack fee is $35.00.
